Cerebral involvement in human immunodeficiency virus (HIV) infection occurs in a significant portion of patients. Despite the presence of viral proteins in brain blood vessels, the mechanism by which HIV gains access to the brain and causes neuronal injury is not well understood. To study the receptor-mediated binding of HIV to endothelial cells, human cerebrovascular endothelial cells (ECV304) and human brain microvascular endothelial cells (HBMEC) were examined for their susceptibility to HIV infection. ECV304 cells and HBMEC from two patients were infected with HIV-1(MN) in the presence of polybrene. The absence of syncytia formation in both cell types suggested the absence of CXCR4 and CCR5 and the presence of an alternative receptor. Analysis of confocal immunofluorescence of both cell types using a panel of anti-HIV receptor antibodies identified the presence of CD4 and CXCR4 but not CCR5 on HBMEC. Likewise, HBMEC expressed ICAM-1 and laminin but not CD24, CXCR3, CXCR5, and the alpha(4)/beta(1)-integrin receptor. HIV-1(MN) gp120-specific monoclonal antibodies (Mabs) selectively bound to HBMEC, suggesting the presence of a novel receptor not yet described for HIV. The binding of gp120 to ECV304 and HBMEC was inhibited by soluble CD4 and CXCR4 or CCR5 Mabs. In contrast, both cells were resistant to HIV infection in the absence of polybrene. Thus, both ECV304 and HBMEC express CD4 and CXCR4 but not CCR5, suggesting that these cells may serve as a model for the investigation of HIV infection and access to the brain..
